Go to your device Settings → Apps, select the app, then go to Storage → Clear data, or simply uninstall the app. This permanently removes all data stored on your device.

Reset or delete advertising identifiers
The advertising ID used by AdMob for ads can be reset or deleted at any time via Settings → Privacy → Ads → Reset advertising ID / Delete advertising ID on your Android device.
The exact menu location may vary slightly depending on your device manufacturer and Android version.
